FREE Assessment

At Total Care Connections, we make every effort to understand your needs in order to provide you with the right care. A member of our team will meet with you one-on-one to outline what is most important to you and how we can provide a solution that will exceed your expectations.


Step Two

Choose your CARE option

In-Home Care

One of our care managers will meet with you to design a customized care plan and schedule based upon your specific preferences. Once everything has been determined, we will introduce you to the certified caregivers who best fit your situation. We don’t require any notice to begin services as we are available 24 hours/day!

Assisted Living

After determining what is most important to you in an assisted/independent living facility, our team will put together a list of communities that score the highest in your area. You will have the opportunity to take a tour of these communities to select the best option. Once a community is selected, we will introduce you to the care staff to make the transition into your new home!


Step Three

Follow-up

Our goal is that we will not only meet your needs initially, but throughout the entire duration of your need for care. Our team will follow-up with you to ensure that you have everything you need and if your requirements change, we will be there to make the necessary changes to continue exceeding your expectations.
